Required to certify that you hold a valid seller permit, issued pursuant to the Sales and Use Tax Law.
Authorizes the use of a credit card for immediate payment of products and services rendered.
Application for payment terms as a method of payment for products and services rendered– can require up to five business days for approval.
Additional form submission is required for the following states:
Alabama, Colorado, Florida, Maine, & Oklahoma: Copy of state business license/resale certificate
Louisiana: Form R1064
Massachusetts: Form ST4
Minnesota: Form ST3
New York: Form ST120
